To qualify for our free mentorship program, you must be 18 years old or older, live in Ohio, and have CareSource, Buckeye, or another type of Medicaid health insurance. If that’s you, please follow this link and fill out the form.

You’ll meet with your mentor via phone calls, or video chat. This allows us to serve clients throughout the State of Ohio. Additionally, this enables us to serve you in a way that is convenient and fits into your schedule.
If you qualify, you can receive up to 3-hours per week of personalized mentorship at no cost to you.
