To qualify for our substance use disorder program, you must be 18 years old or older, live in Ohio, and have CareSource, Buckeye, or another type of Medicaid health insurance. If that’s you, please fill out the form to get started.

We offer comprehensive, Medicaid-funded support for individuals facing Substance Use Disorder (SUD). Our services include personalized recovery plans, one-on-one support, life skills coaching, and access to a supportive peer community through free online and offline events. Each aspect of our program is designed to help you build resilience, manage challenges, and achieve lasting recovery—all from the convenience of home.
You’ll meet with your mentor via phone calls, video chat, or text. This allows us to serve clients throughout the State of Ohio. Additionally, this enables us to serve you in a way that is convenient and fits into your schedule.
If you qualify, you can receive personalized support at no cost to you, fully covered by Medicaid. The amount of time you spend with your mentor will be tailored to your unique situation.
If you require a higher level of care, such as medical detox or a short-term inpatient stay, Evergreen Mentors are here to help. We’ll support you in finding the right providers and resources, and continue to stay connected so that, when you’re ready, you can smoothly transition back into our program.
